   Concussion/Injury Documents and Course

Concussion/Injury Documents for Coaches - please read and keep required documents with you.  The Concussion Training Course must be completed for Fall 2019 through Spring 2020 coaches and assistant coaches. 


   NEW FOR THE 2019-20 Season - Coaches and Assistant Coaches must complete the following:

If you volunteer to be a coach or assistant coach, you must register as a coach in the Penn Legacy system where you register your players. This process will ask some generic risk management questions, and also if you completed your child clearance documents* (See below). If you have already completed your clearances, Penn Legacy does maintain a copy of your documents on file. You also have the opportunity to update your concussion certification during registration.
New this year is the Safe Sport certification. This is mandated training in response to Federal legislation and is not an Eastern Pa Youth Soccer mandate. The course is free for volunteers but will take some time to complete. We appreciate your willingness to comply with ALL federal and state laws regarding keeping children safe.

*Required Background Checks/Clearances and Documentation to Submit to Penn Legacy

  • Background Checks and Clearance Guidelines for the 2019/20 Soccer Seasons - Volunteer Coaches, Assistant Coaches, Program Volunteers
  • Penn Legacy Confidentiality Agreement - to be completed/signed by board members, volunteers, coaches and contractors (good for lifetime of service) - sign, scan and upload to form below if not previously submitted
  • Concussion Course Completion Certificate - Required to be completed and submitted annually by coaches (see above section regarding concussion certification during coach registration). Save a certificate copy to your hard drive and upload to form below.

The Penn Legacy Board of Directors has determined that coaches/assistants will not be allowed to take the field or meet with their team without the required clearances. Once you have completed your clearances, please go to the following web site to submit your status:
